Description:

St. Luke Community Clinic (SLCC) serves uninsured patients who are under 200% of the federal poverty level. SLCC provides chronic and primary care to eligible individuals referring patients for more specialized care if that care is available from our medical partners in our community. From the ages of 18 to 64, our patient population transcends all walks of life. A woman?s health program is available. The clinic also works with patients to obtain necessary medications.

Hours:

Monday through Thursday: 9:00 a.m. to 5:00 p.m., closed from noon to 1:00 p.m.
Monday, Wednesday and Thursday: Chronic Care By Appointment Only
Tuesday: Walk-in clinic for established patients
Thursday Evening: Walk-in clinic at 5:00 - 6:30 p.m. for new patient registration. All new patients must be seen for the first time at Thursday night walk-in clinic.
